The Ministry of Health Wellness and the Environment recently launched a Chronic Care Passport programme as part of its ongoing efforts to address the challenges arising from the complications of diabetes and other chronic non-communicable diseases.
A release from the Ministry said that the passport is a patient-held take-home record that produces a meal plan, encouraging interaction between patients and health care teams.
It is intended to strengthen control and adherence to medication, and its components include: standards of care, results of examination findings and laboratory studies, meal planning and a medication list.
The Ministry said that the ultimate aim of the project is to empower clients with type-two diabetes so that they will be able to gain greater control over their diabetes, through behaviour change and enhanced self-management skills.
